Ontario CCACs Health comes home: a conversation about care: Part 2

Ontario's CCACs are developing a series of four papers intended to start a broad dialogue on how to tackle our common challenges and harness the opportunities – we need a shared understanding of where we are going and resolve to take the journey together. 
Last month, we presented our first paper in a four part series. Now, we are pleased to present the second: Health Comes Home, Part 2: A Conversation about Aging and Chronic Care. It looks to the future - a time when the Baby Boomer generation has turned 80 – and Ontario's health system will need to be equipped to meet the needs of more people with chronic health conditions and seniors with increasingly complex health needs will want to stay at home for as long as possible.
