Health Links is weeks away from sheltering hundreds of the region's most ill patients under a specialized care plan umbrella.
The Quinte-area health care connection program, aimed at preventing chronically ill patients from taxing limited hospital resources, is coming online this fall with $600,000 in funding from the Ministry of Health.
Under the Health Links program, patients battling a host of chronic diseases such as diabetes and congestive heart problems will be plugged into a care plan accessible to their varied care providers, said Dr. Jonathan Kerr, primary care lead for South East Local Health Integration Network.
