Speak Up TODAY for Advance Care Planning

MESSAGE FROM THE SOUTH EAST PALLIATIVE PAIN & SYMPTOM MANAGEMENT CONSULTATION SERVICE

It’s here!  April 16th is National Advance Care Planning Day -- a day to spread the word about the “Speak Up” program developed by the Canadian Hospice Palliative Care Association (CHPCA).

“Speak Up” is designed to raise awareness about end-of-life care so that individuals, families and health care providers can make informed choices.

“Speak Up” highlights advance care planning -- a process of reflection and communication about health and personal care decisions that might surface at the end of life.  It’s a way to start thinking and talking about what’s important to you.

  *   Take a few minutes to watch the “Speak Up” videos
  *   Check the online “Speak Up” workbook (it's printable)
  *   Tell a friend, family member or client about “Speak Up”

It's time for every Canadian to learn how communicate their wishes for end-of-life care.
