Case study: #25/26/27/28

Case study #25

 An 80 year old male with multiple myeloma and was on dialysis.  He discontinued dialysis.  His 84 year old wife needs more support.  She has just spent 4 days in the hospital with angina.

#26.    A 57 year old woman suffering from multiple sclerosis has been very isolated.  She moves very slowly.  She said it took her a long time to get the courage  to call for support. She needs a friend.

27.    A 93 year old woman with multiple health concerns (heart disease, arthritis, very frail) would also welcome a weekly visit. 

28. A 79 year old woman needs supervision, while her 83 year old husband goes out and runs some errands.  They have a small dog and they both smoke.  She has had triple bypass surgery, a stroke, and fractured both hips. Her first hip fracture happened while she was in the hospital and has resulted in the decision to stay in the home.  She is in a wheel chair and uses a walker to transfer for toileting.
